Primary Purpose:
Facilitate the overall implementation of transition planning and services, assuring that coordinated outcomes are appropriately planned for all students with disabilities.Provide parents and teachers with appropriate transition resources necessary to effectively plan and deliver transition services.
Qualifications:
Education/Certification:
Bachelor’s degree from accredited university Valid Texas teaching certificate in the field of special education
Special Knowledge/Skills:
Knowledge of federal laws and state requirements related to special education students and programs
Knowledge of the characteristics of various handicapping conditions
Ability to function within school and community settings
Ability to implement appropriate accommodations/modifications to the job setting for individuals with special needs
Strong computer skills
Ability to work cooperatively with staff, students, parents and the community
Knowledge of current regulations governing transition services for students with disabilities
Ability to work with various community and governmental agencies
Ability to interpret data Excellent communication (written and oral), organizational, and interpersonal skills
Ability to effectively develop training materials and provide professional development to district staff
Comprehensive understanding of graduation requirements as stated by TEA
Experience:
Three years of teaching experience
Major Responsibilities and Duties:
Assessment
1. Collect and organize relevant data from student’s cumulative folder, the student, classroom teachers(s), principal, support staff, parents, and outside resource people.
2. Conduct classroom observation and personal interviews.
3. Participate in the Admission, Review, and Dismissal (ARD) Committee to assist the development of transition/graduation plans for student Individual Education Plans (IEP) according to district procedures.
Consultation
4. Provide staff development training in assigned schools to assist school personnel in identification and understanding of students with disabilities.
5. Assist teachers in identifying student interests and transition needs.
6. Consult parents, teachers, administrators, and other relevant individuals to enhance their work with students.
Program Management
7. Participate in IEP development and placement decision making.
8. Maintain documentation as required for compliance with timelines and other procedural requirements.
9. Schedule and facilitate preparations for ARD meetings including completion and/or collection of appropriate paperwork.
10. Work with the student, the student’s parents, school officials, and other agencies to enable the student to transition from school to productive adult life.
11. Establish and maintain a network of community resources and employers willing to assist with job experiences.
12. Assist teachers and/or Director of Special Education in developing overall plan for the district’s transition program.
13. Maintain data on district transition activities.
14. Coordinate parent involvement activities for transition.
15. Compile, maintain, and file all physical and computerized reports, records, and other documents required.
16. Comply with policies established by federal and state law, State Board of Education rule, and local board policy in the areas of assessment, placement, and planning for special education services.
17. Comply with all district and local campus routines and regulations.
18. Participate in professional development activities to improve skills related to job assignment.
19. Coordinate with Counselor’s office regarding student’s schedules, credits and transcripts.
Communication
20. Maintain a positive and effective relationship with supervisors.
21. Effectively communicate with colleagues, students, and parents regarding special education services and student needs. 22. Maintain confidentiality.
23. Observe professional and ethical standards, in accordance with generally accepted community standards and the Professional Educators Code of Ethics
24. Perform other duties as assigned.

25. Participate in staff development activities to improve job-related skills.
26. Attend and participate in faculty meetings and serve on staff committees as required.
Supervisory Responsibilities:
Take all necessary and reasonable precautions to protect students, equipment, materials and facilities.
